PQRI is a program of the Centers for Medicare and Medicaid which provides information about certain standard of care measures. The program was started in 2006 by Congressional Act and made permanent in 2008. The initial act provided monetary incentives for medical providers who participate but this money is only assured through 2010. The complete rules and regulations are beyond the scope of this article, but initial information about the program follows.

The PQRI relies on 9 different reporting options based on the time period of reporting, claims based reporting, and the measures which are reported. The measures are standards of care which are updated each year. The Centers for Medicare and Medicaid web site provides additional information about the program an is updated every year. Additionally, the American Medical Association’s web site, specialty medical board web sites, and most state medical societies have information about the program.

Though only authorized through 2010, a two percent bonus is provided to physicians who participate. Participation is voluntary and has not been universal. Many reasons have been cited including increased expense associated with manpower required for reporting, inability to incorporate reporting codes with billing systems, and the lack of return for smaller practices who may not have a large enough Medicare enrollment to warrant participation.

As an additional part of the reporting, providers can also earn an extra two percent for reporting of prescription habits. There are three codes which can be used for prescriptions and the CMS web site provides information for physicians on this part of the program as well.

Quality improvement in medicine is an idea which has been around for quite some time. Most, if not every physician agrees that quality of care is a priority, determining what constitutes quality is a much more difficult problem.
